Records Request
All patient care records are confidential and protected by privacy laws including HIPAA.
The Ridgewood Fire Department has established a privacy and security policy to comply with these laws and to assure the integrity of your protected health information.

All requests for copies of records by the patient must be made utilizing the Village of Ridgewood HIPAA Authorization Form.
This form needs to be either:
a. notarized
b. Submitted in-person, via e-mail or regular mail with a copy of the matching Driver’s License or State-issued ID.
Requests by attorneys should include a signed HIPAA or medical release form equivalent to the Village of Ridgewood HIPAA Authorization Form.
Only under a few exceptions, such as complying with a subpoena, can medical information be released without the consent of the patient.

All requests should include a contact phone number for verification. Please indicate how you would like the report sent to you (mail, or email.)
Records requests will be completed within 30 days of receiving all signed paperwork. A small fee may be charged in accordance with applicable NJ laws.
